And peace be upon you and God's mercy and blessings.

Grand Ayatollah Sayyid Khamenei (may God protect him):

The worshiper should intend to perform the prostration of forgetfulness immediately after the salutation, by prostrating on what is permissible to prostrate on. It is recommended to say: "In the name of God and by God, peace be upon you, O Prophet, and God's mercy and blessings." Then he raises his head from the prostration and prostrates again, repeating the same remembrance. After that, he raises his head from the prostration, recites the Tashahhud, and concludes with the salutation.

Grand Ayatollah Sayyid Sistani (may God protect him):

The prostration of forgetfulness consists of two consecutive prostrations, and it requires the intention of closeness to God. There is no need for Takbir (saying "Allahu Akbar"). It is strongly recommended to place the forehead on what is permissible to prostrate on, and it is also advisable to place all other parts of the face that touch the ground and to observe all conditions required for prostration in prayer, such as purity, facing the Qibla, and covering oneself. It is also recommended to recite the remembrance in each prostration, preferably: (In the name of God and by God, peace be upon you, O Prophet, and God's mercy and blessings). After raising the head from the second prostration, the Tashahhud must be recited, followed by the salutation. It is strongly recommended to choose the common Tashahhud rather than the longer version.
