Grand Ayatollah Sayyid Khamenei (may Allah protect him):
A1) It is forbidden to face or turn away from the Qibla while relieving oneself, and it suffices to deviate slightly to be considered turning away from the direction of the Qibla.
A2) It is forbidden to turn away from or face the Qibla with the front parts of the body, which include the chest and abdomen, even if the private parts are directed away from it. The criterion is the common understanding of facing and turning away, and it seems that the knees are not included in this. It is recommended to avoid facing the Qibla with the private parts only, even if the front parts of the body are not directed towards it.
