Grand Ayatollah Sayyid Khamenei (may God protect him) and Grand Ayatollah Sayyid Sistani (may God protect him):
If you are frequently in doubt, do not pay attention to your doubts. If you doubt whether you performed one or two prostrations, you should assume that you performed the second one.
However, if you are not frequently in doubt, and you have knowledge or certainty (a belief close to knowledge) that you are in the first or second prostration, you should rely on that. If you have doubt or suspicion, you must prostrate if you have not exceeded the place of prostration; if you have exceeded it, then there is nothing upon you.
Illustrative Example:
If a person doubts while sitting on the ground and before starting the Tashahhud whether they performed the second prostration or not, they have not exceeded the place, so they must prostrate.
However, if they doubt during the Tashahhud whether they performed the second prostration or not, they should not pay attention to their doubt.
